Appointments
To schedule an appointment, please call the number to the office closest to you, or fill out the appointment request form.
Canceling or Rescheduling Appointments
If you need to cancel your appointment or reschedule for another time, please call the office nearest you at least 24 hours prior to your scheduled appointment time. We will do our best to accommodate your preferred date and time.
New Patients
If you would like to become a new patient of StoneCreek Dental Care, call your local office to set up an initial meeting, cleaning, and evaluation. You will need to fill out a new patient form and present it on the day of your scheduled appointment. You can either complete the form when you arrive, or you can download a printable PDF version and complete it prior to your visit. You must fill out ALL pages of the form.

Insurance and Financing
StoneCreek Dental Care accepts all major forms of dental insurance, including Aetna, BlueCross BlueShield (BCBS), Careington Benefit Solutions, Cigna, Delta Dental, Dentanet, DenteMax, GEHA Connection, Guardian, Humana, Metflife, MHBP, Southland, Tri-Care, United Concordia, United Healthcare and many others. Please call to find out what your benefits are.
We would like to stress, however, that you should not let a lack of insurance affect your oral health. We offer financing and payment plans to accommodate patients who are uninsured.
Home and Post-Treatment Care
In order to maintain good oral health and prevent cavities, gum disease, and other dental complications, the American Dental Association recommends you brush your teeth twice a day with a soft-bristled toothbrush and an ADA approved fluoride toothpaste. It is also important to floss daily and avoid excess amounts of sugar, acidic beverages, and smoking.
If you have recently undergone a dental procedure or received some other form of dental treatment, it is important that you follow ALL post-treatment guidelines given to you by your dentist. Failure to do so can result in a number of complications. If you have questions about any post-treatment care, do not hesitate to call your local StoneCreek Dental Care office.
